Are you having a plated dinner? If guests have to mark their meal choice on the RSVP card, I would just leave room on the card for the person name and meal choice, like this:
Mr./Ms. _____________________________
Chicken_____ Beef_____ Vegetarian______
If you are having a buffet, maybe you can still put it on the RSVP card by leaving room for everyone's names and a check to say if they accept the invite or not, like this:
Mr./Ms. _____________________________
Accept______ Decline______
I agree. Let them fill in the name of their guest.
It is standard practice to just let the guest fill in their names on the RSVP card. If however you get one back without the guest name, then feel free to call them or email them. But you will not likely have many issues with this.

If you are close with the guest, I don't think it would be an awkward call or an email just to get the name of their date. They may appreciate you including their +1 as something other than "& guest". It if is a invitee of your parents or FI's parents, they could make the call for you.

For most of our guests we know their SO's name but there are a few we do not. I need to know their full names for our escort cards but I'm not sure how to ask for their name without it being awkward or weird. Should I enclose a note with the RSVP card? Any ideas are appreciated!!!
