Weddingbee Boards

We are inviting families with children to our wedding.  How can we have people indicate the following on the response card?

 # adults

# children

# who want chicken, fish, or require a kosher option.

I am assuming that vegetarians order specially at the wedding, or how is that done?  I have never seen that option on an rsvp card.  Should I add that, too?

Also, we want to know how many children because we are serving a special child's meal.
