I don't think I've seen this. I have seen menus at each guests plate, but not serving AS the place card. Sorry I'm not more help.
I haven't seen this but since we're having our ceremony cabaret-style, we'll do menus/programs togeter, and I'd like them to have the names on them. I was considering just doing a clear label with their name in a nice font.
I haven't seen anything like that in person or online but I did come across something that could be adapted into a placecard. This is a menu/favor combination:

Hi Bees!
I've seen a few pictures of menus that doubled as placecards, and they looked really cool. However, they also seemed quite fancy and hand-calligraphed. I'm not sure if I want to spend our budget on hand-callighraphing our guests' names on menus - so I was wondering if any Bees out there have done this or seen this. If so, how was it done? Was it hand written, calligraphed, printed on, or something else I'm not thinking of?
