I would think after the shower. Thank them in front of everyone but present the gift privately.
The last shower I went to was a baby shower that was thrown by the mommy-to-be's 2 sisters. After the last present was open, the mommy to be thanked everyone and said she had something special for her sisters for throwing her the amazing shower. It was actually really sweet and the 3 of them cried. I was planning on giving mine after the shower but once I saw this I decided I would do the same.
I still have not given them the gifts.
I did not have them ready and have not seen them since the shower since everyone has been on vacation.
For two hostesses, I sent flower arrangements to their homes in advance of the showers. One hostess received flowers after the shower (the next following business day). And one hostess received a thank you card with a gift card inside, mailed a few days after the shower.

Curious when the best time to give a shower hostess a gift. Is it when you first arrive and have gifts in your hands? Or, better to wait until the end and profusely thank them and leave them with their gift?
Thoughts?
