I second the diapers! You really can't stock up enough on those things! I would just do a box of diapers, some wipes and a cute outfit or two.
I have two go-to gifts right now...
One is Aden and Anais swaddle blankets (soft and amazing and can never have too many)
and the other is Trumpeter socks (they have adorable ones for boys. I've been buying the argyle).
These are both things that moms would tend not to splurge on for themselves but love when they get!
A Baby Milestone book where you record their firsts. Each kid needs one...don't want to leave out the younger kids. I like the Trumpeter socks I got...they ARE very nice.
I've been gifting new babies a little basket of some of my favorite things: SwaddleMe blankets, Aden and Anais blankets, Munchkin teething animals, hooded towels, and diapers.com gift cards.
I usually get the new mom a spa gift certificate. I know it's not really a baby gift but who couldn't use a hair cut or something to make them feel a little special. You can always pair it with a cute outfit to make it more of a baby gift.
I like these too because often 2nd babies don't get as many pictures etc as the first since there is not as much time. These are a cute (and quick!) way to capture the baby's first year
I would want diapers... maybe a different diaper bag and clothes are always fun to get

A good friend just had her second baby (a boy). Her first born is also a boy and is 3 years old. Because of her first born, she has all the boy baby things she needs (the first baby is spoiled rotten). I'm trying to figure out what to get her for baby #2, but I can't think of anything she'd need or want. She isn't registered anywhere, so no help there.
