Someone posted a link to one on here that I downloaded. It's a pdf file. I could email it to you if you PM me your address.
I like it all to be paperless so I've created a planning notebook with evernote.com and it has an app for my phone so I can pull up all my notes and pics when I'm out and about.
I saw this program on A Practical Wedding and I've just started playing with it. Seems pretty good so far! It's wed based, which is a big plus for me planning with a long distance partner.
