Are those are the papers your parents sign to say you aren't married? If so, in my archdiocese we aren't required to have them. My priest said a verbal answer was enough. Sorry I can't help! I would recommend calling him and telling him you've misplaced them, if you've already googled, etc., I doubt you're going to find them. Good luck!
My fiance's sister who was married in July had to complete them. It was a simple form.
During my marriage prep meetings with the priest it wasn't mentioned. So I assume we don't have to do it.
Our priest met with our parents for about a half hour per set of parents right before the rehearsal, so we didn't have to fill out forms either. Maybe you can call the priest and ask if that would be ok or if he can give you another set of copies. You don't have to be afraid of your priest - after all, he will be in a good bit of your wedding photos!

We are getting married by our deacon. He has reminded me several time about "letters of consent" and even given me copies.
I keep forgetting to have our parents fill out the letters of consent. Now I can't find them and haven't found the template online... any suggestions?
Did you Catholic girls have to fill these out too?
