We did the FOCCUS course through a counselling service near us, I've just had a look online but I'm pretty sure bc of how it's done you need a counsellor to do it with you (can't just download the questionnaire).
I've heard people talk about a book called "100 questions to ask before you marry" or something like that, maybe that would be worth looking for?

Hi hive,
We are getting married by a friend who happens to be a notary so there isn't like a priest to do any sort of premarital counseling--and it is not in our budget to pay anyone. However, I would still like to have a date with my FI and discuss our life and relationship in a meaningful and pointed way before the wedding. We talk about this kind of stuff PLENTY and we don't have any serious problems, I just saw one of the bloggers (Seahorse I think?) talk about how she went through some premarital worksheets and it seemed like a good idea. However, she mentioned that the worksheets she looked at were specifically for same sex couples. Has anyone come across this for heterosexual couples?
