Could you maybe put headers that say "attending" and "entree selection" and then a few lines below so they put their specific entree beside their name? That's all I can really think of at the moment.
The last invite I received said something like:

__ are attending / __ are not attending
each guest, please initial your meal choice

__chicken
__fish

I am printing my own invites. The venue I am using needs me to tell them who is getting chicken, beef or fish on the place cards. What should I put on the rsvp cards so they will tell me who is getting what entree?
