You have to check out high end decor/event rental places in your area...but this looks like it might be unique to one company and may be something like 10,000 to get it to you and back.
If you aren't dead set on the chandelier, it looks like it'd be fairly easy to make with thick plexiglass. In fact, it looks like each table is 2 pieces....a table part and then an elevated base for the chandelier that fits inside the table part.
