I would contact the parish that the baptizing priest was at - at the time of your baptism. The parish should still have records of your baptism. All you need to do is call the rectory and they will direct you to the person who handles all of that - it is usually never the priest anyways.
My fiance had to do a similar thing, his parents moved from the area when he was a baby and the parish had all that information. It was not a problem at all.

Just call them up. They will probably just need your name and baptism date. Technically I think your other sacramental info should be on your baptism certificate, but mine was never put on there, and I know that has happened to other people too. So you should also ask about that when you call them.
I agree with the other ladies here - just make the call. My mom didn't even remember the name of the parish where I was baptised; I had to ask my cousin who still lives in Florida (where I was born). No questions asked, they mailed the certificate right away at no cost to me.
Lol, I just made the call, and the church that baptised me was super friendly and awesome. The woman there was very helpful and gave me some very helpful advice for the rest of the marriage prep process. My other sacraments weren't listed, so she told me to call the parish I got confirmed in for my confirmation certificate. I went to the same catholic school for 10 years and got first communion and confirmation through the attached parish. I just called them up, and they were so much more difficult, which I wasn't expecting, since I know practically everyone there, haha.
At least both documents will be on their way to me soon! Thanks for the reassurance!
How funny that the way that worked out! Luckily, it DID work out!
I called mine up and luckily they HAD received all the notations. Everything but my confirmation was there, however. The funny thing is we left that church RIGHT after my first communion to go to the parish I went to school at.... and then left that one for the one that was new and being held in my public high school! haha

Hey Bees! I'm feeling overly nervous about getting an updated copy of my baptismal certificate (since it has to be issued within 6 months of the wedding).
See, I was baptised the day I was born, while still in the hospital. I was super sick and my mom wanted me baptised before surgery, which I luckily survived. So we just had the random priest who does rounds at that hospital do the baptism. I have the name of the parish that he was from since it was written on the baptismal certificate, but I feel weird about calling them up since I have no further relationship with them. All my other sacraments were done in my parents' parish.
Am I being ridiculous? Should I just call them up? Going in person in a pain since the parish is way up in upper manhattan, and I live in brooklyn and I'd be spending almost 2 hours on the train to get there.
What information will they need from me? Do you think the parish I grew up going to would have sent them my other sacrament information?
