A big box of diapers... they will love you for this.
I like the gift card idea because then they can get something they really want for the baby
Bibs and diapers are good! A friend was going through all of her 1st child's baby close when she was getting ready to have her second child, and she said that all of the bibs were stained. I guess you don't notice as much when your child is 1.5 or so and eating and making a mess everywhere. But she said that the bibs were a little too dirty and stained so she wanted new ones.
And diapers. Diapers are so expensive. Never underestimate the power of diapers as a gift! It's not fun... it's not sexy... but it is appreciated!
A gift card to the local grocery store would be nice. That way they can pick up diapers OR dinner, depending on what they need.
Are you close enough you can volunteer to watch their older child for an hour or two? I think this is what I would most appreciate most: someone to take Addie to the park for an hour and give her some one-on-one attention while I take a much needed rest from my energetic toddler. :)
@skibobrown: I am a monogram type of girl. I will maybe buy a few nice blankets and have them monogrammed with the name. Its very personble, cute and classy.
Something little for the baby and a gift of TIME.
If you could look after the older child, it gives Mom a chance to sleep or have a bath while the baby is sleeping.
Maybe top it off with a casserole for dinner.
With our second we got acouple gift certificates for local food places. That way one of could run and grab food quick and our oldest would be fed. The first couple days were so hectic that making sure our oldest was fed a decent meal, but that we didn't have to cook was nice.
Diapers, if you know they are using disposables are always a great gift! They more then likely still have some toys and clothes from the oldest, so I would stay away from those.
Gift card to The Picture People or another similar portrait studio.
I forgot one thing! If the first child is still quite young, a little gift for them too. It's hard for them to go from being the center of attention and recipient of all gifts in the household to second fiddle.

Our friends just gave birth to their second son today! We want to get them something, but I'm not sure what would be a good gift for second time parents, since they probably have most of the necessities on hand already. Any good ideas?
