I love the madlibs idea, and would go with either of those 2! The guestbooks I've seen with these have been SO fun and i'm sure it would be a hoot to go back and read them again! Have you thought about a polaroid to put a photo of your guests on their madlibs pages? That could be fun too!
@pengoala: that's a great idea!!!! how would the legistics work with it though?
I like the madlib idea. People can have a lot of fun with those. I have seen some variations that the story include advice to give the bride and groom and that can be quite comical.
The thumbprint guestbooks are quite popular too and that can be used as artwork in your home.
@muzikfreek13: So, my husband and I did a photo guestbook at our wedding. We bought one of the fujifilm instax polaroid cameras and the guestbooks with slots for the photos. We had some of my cousins stationed at the guestbook table, taking photos of our guests (mostly in groups, by party or so), and inserting the photos in each page. The guests were then able to write a note for us on each page. It worked out pretty smoothly for us!
This all being said, I imagine it'd be easy enough for you to get the same guestbooks, and print your madlibs pages and stick them on each page in preparation for photos to be added in. I hope this makes sense!
Here's a couple links/photos to what we used:

From our wedding! As you can see, lots of space for stuff to be written, so I imagine it'd be easy for you to do so!

I never understoond the concept of a traditional guestbook - one where guests simply sign their name and move on. To me, it's boring and useless. What am I going to do with a book of signatures in it, I'm never going to look at it again!
So sidestepping the norm, these are the ideas I've come up with. Please let me know what you think, if you think the schematics would work on the day of the wedding, or post your own ideas! I'm all about unique and creative spinoffs from the normal wedding traditions!
1. Make a yearbook style photobook of us over the years we've been dating. (We've been together 10 years and met in 8th grade so we would have pictures of us from when we were 14!) I would leave room on each page for guests to sign and leave wedding wishes for us. I would leave this at the door for guests to sign as they enter.
2. Create a mad lib for guests to fill out including blanks for them to mention thier name, where they traveled from, wedding wishes and something funny as well as ohter areas for them to personalize it. I would leave these at each place setting and have them fill it out during down time (before or after eating, etc). I would put a note stating that this activity is our version of the guestbook and to please drop it in our card box once finished. I would then scan each mad-lib and include it in our photobook. (I'm making the book - our photographer has graciousy given us the copyright to the pictures she takes) In doing this, I only have 1 book for the guest book and pictures, that way I'm more likely look back at the "guestbook" rather than it sit on the shelf and collect dust.
3. This idea is similar to #2 but instead of a mad-lib I would just make note cards for each guest to fill out. name, distance traveled/location, wedding wishes, how do you know the couple, how long have you known them, etc. I would include a note that this is our guest book, please put it in the card box when finished, etc. I would also scan these and include them in our pictures from the wedding.
Please let me kow what you think or post your ideas!
Thanks so much for your insight!
