You could make your own really easily. Just go to OfficeMax and get an index card box and some blank dividers. Then you can have just the sections you need.
Kate's right, you can make your own and it will give you the ability to customize it to suit your needs. Or you can just create an Excel spreadsheet.

I was hoping that someone could give me an recommendation on where to get a bridal file. My sister had one for her wedding several years ago - it's an index file box with printed cards for the guest list with space for names, addresses, gifts, rsvps, etc. I know that I can do this information online, but I like to have a hard copy record that I can sort, etc. Has anyone out there purchased one recently?
