They should customize it for you, might just want to send them a message and ask. Or have someone you know that can paint/draw well make the tree for you (thats what we did, they made it as a gift, so didnt have to pay for it)
Could you make your own? I also ordered from Etsy (and it wouldn't hurt to see if they'd customize based on your ideas), but I love the idea of the paper cut out leaves. I found this on paper leaves:

Or...the other thought is to order the tree with the branches without the deer and add them yourself. And then make your own paper cut out leaves...
@HeatherShane: My MOH is an art and design major so she could probably do it! What kind of canvas did your friend draw on?
@pengoala: I think I'll ask someone it Etsy if they could possibly draw one for me the way I'd like if my MOH can't do it.
I think your inspiration picture was painted by the same person who did ours. She is a seller on etsy...

@Mrs.tobe: Great!! I'm excited, my fiance wanted a slab of wood for everyone to sign that we would then later hang on the wall.....I see where he thought that would look okay but I want something a little more wall-worthy. Thanks again for your help :)
@nrieder: Funny you should say that, we were going to do that then switched to a bourbon barrel head. So we actually had both! It went over fantastic!
ETA: This is the barrel head that we used after we got our WP to sign it at the rehearsal dinner. It's completely full of signatures now. :) 
@Mrs.tobe: Oh haha, that's actually really cute! I think if we were to do something more like that I'd like it better. Maybe I just didn't like the way the wood looked when it was cut, i dunno.
@Mrs.tobe: Do you remember how big yours was? She quoted me for $95 saying I would need one that big for 150 people but yours looks like it would work just fine.

I'd like to use this as an idea for my guest book, but I want deer instead of birds and I wamt orange and red (fall colors
) paper leaves instead of finger prints. I have no idea where to order one though. I've seen several on etsy, but can they make mine the way I want? Or do I have to use their template? I'm in love with this tree, a lot of other trees on templates I've seen aren't ones I like. Anyone know where can I order one?
