I haven't, but I noticed a few people on Etsy who also offer the service.
I'm using wedding mapper also. That way our guests can see everything in relation, plus find fun stuff to do besides the wedding!
Yes, I've done them for people before. I used Illustrator. But I've seen people do them with gocco's and even through google maps.
There was a bee that did it in Adobe Illustrator. It turned out really well!
It actually isn't that hard to do- take a screenshot of a map, and trace it using any draw program that is slightly more advanced than MS paint. You can even do it in MS Word (or powerpoint, I think). Trace over the roads, and then delete the original map. You will be left with your own map.
we used google maps because most people are familiar with google maps (or a similar online map like mapquest). they can easily see where things are relative to each other & we can update it as we go. guests can lookup other things to do in the city, and easily type their address in to see how long of a drive it is. oh and it's free & you can change the markers, add as much info as you want about the location..very custom. the link is on our website & our easy to remember website link is on our Save the dates & invites.
the only problem with google maps is the link name itself, if you're writing it out somewhere, is incredibly long & impossible to remember or fit on anything (which is why it's just a link on our site). in our case, the invited people who arent comfortable online probably wouldnt want to use a cutesy map anyway & would just find it on a street map.
This is a bad picture, but I also did mine in Illustrator. Same directions as dahlia mentioned, but you can make the lines smoother using the Warp Tool. I loved doing it so I'm doing several others for some fellow knotties.

I designed wedding bee "bettina"'s maps for her. I just finished them for her and she loves them.

weddingmapper uses Google Maps and is free. you can also use past brides maps to help you plan your wedding. I'm hooked!
