DH plays lacrosse. He wound up with a bruise on his cheekbone near his eye a few days before he had to stand up in our friends wedding. I had to cover up the bruise with concealer so it wouldn't stand out and ruin their wedding photos. That is the only time he's ever worn make up.
Not makeup, but we had an ice storm once and lost power for 13 hours, so he let me paint his fingernails all different colors to kill time! ;-)
EDIT - He did let me put concealer on him once when he had a nasty bruise.
I have begged on multiple occasions to let my husband put just mascara on him because he has the most gorgeous eyelashes on the planet, but I still have not won that request. :(
I didn't answer the poll because "No he refuses to let me" didn't work for me because I have never wanted to put make-up on DH nor has he ever wanted to do my make-up!
However, I have coloured in his tattoo with sharpie's!!!!
I haven't done the works, like eyeshadow and such, but if he has a nasty zit he'll let me put a LITTLE concealer or powder on it, just to take some of the red away.
I probably wouldn't let him put make up on me for fear of looking like a serious clown.
@Future Mrs. Martin: Ha, I have always wanted to do that!! I let my sister color mine in with Crayola markers, but it's on my back so I can't do it myself :(
@deetroitwhat: Lol, me too!! DH has such pretty lashes, but they're blond so they don't stand out. I just wanted to see what they'd look like "enhanced a little"!!
Yes he let me, but as a clarifier, we are theater people. For Halloween he was Abe Lincoln and I needed to age him as he is only 23. It was a lot of fun and he did put up with a few trial runs. I know that stage stuff would be the only reason he would let me put make-up on him.
In regards to him putting make-up on me, he actually doesnt' like when I wear make-up so I really don't see that happening.
I might have to let him try that sometime! lol i'm super interested to see how he would do, too bad i know I'll probably never be able to do that on him.
funny thing though just last weekend he helped me straighten my hair. he was so concentrated on making sure every single hair was done, it was uber-cute! 
haha no i dont think he would ever let me but let me tell you, the man knows how to color some hair (i make him help me out from time to time - my hair is just getting too long and i cant do it alone)
but i did put eyeliner on one of my ex boyfreinds once - not going to lie, it was kinda hot haha but he should never leave the house like that
The day of our e-pics, DH woke up with a couple of zits so he let me put a little of my concealer and powder on them.
Outside of that, I have no real desire to put make-up on DH nor does he want to do my make-up.
Sometimes when I don't feel like moving from the couch I make DH actually apply chapstick on my lips :) That is as close as we've gotten to making each other up.
Not make up but he did try on one of my bikini's one time to make me laugh. Love that guy!
He's a photographer and I occassionaly model for him, which involves quite heavy make up. He once had a go at putting it on me - I think he was pretty shocked at how hard it was! He has also helped me with false eyelashes a couple of times (I really can't do them myself!!!)
He has never let me near him with make-up, but he has long hair so I think he'd worry it would make him look girlie...

Last night I let DH do my makeup. It was mostly for fun, but I also secretly wanted him to know that it's not that easy, and to understand why I can't always be ready to go out at a moment's notice!
He actually did surprisingly well! I showed him all the different types of makeup and told him where it goes, which brushes to use, and let him go to town. The only things he really had trouble with were the gel eyeliner (he put a big thick blob in the inner corner of my eye, and then the line sort of meandered up toward my eyebrow), and the blush (he put it over my whole cheeks, from jawbone to cheekbone). But overall he was quite pleased with his work. :)
He will not let me put any makeup on him though! Do your SOs ever let you do them up? I think DH might be a bit traumatized after his mom and his aunt experimented on him and his cousin when they were kids. They tested hair bleach and fake tanner on DH and his cousin, causing them to look like orange boy-band rejects. Ooops!
