I know I saw the directions for these on Martha's website not too long ago because I was considering doing them myself ... have you tried a search through martha's website, rather than just clicking on the link? I've noticed that it can be tough to find certain project on her website so you might have to dig for a while.
FWIW I think it wouldn't be too hard to figure these out ... you'd just need long strips of cardstock, which you'd fold accordion style. Then punch a hole at the bottom corners and run the ribbon through.
I searched martha's site and came across the picture, but when I clicked the 'how-to' it said they had been removed!
I agree with Editrix - with some experimentation, you should be able to figure these out in short order. Perhaps mock it up with plain paper and string or curling ribbon until you figure out the template that would work for the size card you would like to use?

I'm in love with Mrs. Lime's escort card holder (found here), but I can't find the tutorial! Since its a few years old, I think Martha's website nixed it. Anyone know the how-to's of this cute idea?
