I would suggest sticking a bit closer to the standard wording:
"M___________ / __ accepts / __ declines" (the "M" being where people should write "Mr. & Mrs. Jones"; some people replace "M" with "Name(s)").
Since you have the names, adding a line for "number of guests attending __" or "__ seats have been reserved in your honor" is optional. I'd only add it if I were expecting people trying to add to the guestlist.
And I would definitely ask people to initial their menu choice- if a family of four makes different choices, how would you know who wants to eat what? "Please initial you menu choice / __ beef __ chicken __ fish __ pasta".
Are you planning to write your guests names on the RSVP card yourself?
I put please initial each guest dinner selection... so far so good. I have gotten back like half my rsvps
Having guests initial each entree choice is a good idea. I'm glad I saw this post, because that's one detail I overlooked.
Ya it's a good idea if they would actually do it... 90% of mine didn't initial and just checked the boxes... which is fine if they both want the same meal... but many times there's one of each and you odn't know who wants what! Oh well! they're fault.
Oh we also had one that didn't put their name on it at all. I have seen a suggestion on here to number them in the corner and record who got send which numbered card... I didn't thik anyone would send one back without a name on it, but low and behold!

I think what I may end up doing (because I'm going to attemp to do the invites myself) is:

Obviously I could word it different, the most people on each card will be two because we are having an adult only ceremony/reception. Do you think this is ok, or is it too repeative?

I can't wrap my head around how I should word the RSVP/DINNER SELECTION cards. I am doing them together on one postcard to make it easier and save money. We are very limited not only in space but budget as well and can only let guests in serious relationships bring dates. I know to address the invitations to everyone invited, but how do I word this:
Jane Doe & John Hanckcock
_YES
_NO
_number of guests attending (if i put this will people assume they can bring more then listed?!)
MEAL SELECTION:
_BEEF
_CHICKEN
_FISH
_PASTA
(then when they select these options how do I know who wants what?!)
should I ask them to initial next to the meal each one wants?
PLEASE HELP! I AM WEDDING STUPID!
