I found this posted by another bee on here and saved it because I liked the wording, the fact that it was all on one card, and not to mention super cute! It also allows you to "write in" the # of seats you are allotting to that guest (which I think is good because people tend to overstep their bounds, given the chance). Like I said this is not mine and I don't remember the initial bee but hopefully this will help you:

@texmax: hi wedding date twin! i had a hard time with this decision too. my venue needs to know who is having which entree (not just which table #) so this is what i came up with for the replies.

I can't wrap my head around how I should word the RSVP/DINNER SELECTION cards. I am doing them together on one postcard to make it easier and save money. We are very limited not only in space but budget as well and can only let guests in serious relationships bring dates. I know to address the invitations to everyone invited, but how do I word this:
Jane Doe & John Hanckcock
_YES
_NO
_number of guests attending (if i put this will people assume they can bring more then listed?!)
MEAL SELECTION:
_BEEF
_CHICKEN
_FISH
_PASTA
(then when they select these options how do I know who wants what?!)
should I ask them to initial next to the meal each one wants?
PLEASE HELP! I AM WEDDING STUPID!
