mark it out with a ruler where you want the slits and how long are you going to make them to fit your ribbon. Then use an exacto knife (one of those fancy craft knifes, or if you don't have one a box cutter even a realy sharp pen knife will work) to trace the cuts and erase the marks. Ta da!
I helped make something similar to this for a friends wedding to hold favors, the ribbon is supposed to in theory hold it together, but we used a thin strip of double sided tape and it worked great.

It would be much easier to make the slits if you find a paper punch the size and shape you need.
