I would leave out the "written by" part and just put "Performed by John Smith"
In my program, I would have written it as...
Ave Maria / Franz Shubert
Performed by John Smith
Usually you leave out both "written by" and "performed by" - you can put the performer's name in italics or boldface underneath. Make the title left-justified, composer/songwriter right-justified, and performer's name centered on the next line. This is pretty "clean-looking" and is how it's often done in concert programs, which is I think the format you're looking for.

Four songs are being played by my ceremony, from Johnny Cash to Beethoven, and they are being performed by a friends/family.
So, how do I list this on the program?
For example:
Ave Maria, written by Franz Schubert, performed by John Smith
(That doesn't seem like it would look good)
How is this done typically?
