My guess is you tie it like a regular knot but slip in an extra piece of ribbon into the loop just before you tighten the knot.

It looks to me like they tied a square knot, then made a bow with only one loop. Good luck.
Hmm...could they have made a BIG bow then cut the loop down the middle on each side and recut to make pretty?

If they had made a bow and cut it, then the bottom ribbon would show the inside of the ribbon. It looks as if both layers show the same side, so I vote for a square knot with another piece of ribbon slipped through.
