I wrestled with this for a while. There are some very well-designed RSVPs out there, but I didn't want our RSVP cards to feel like a form. We're seeing how it goes, but I know that some people are bound to do this. I asked for their email address on the RSVP, so hopefully we can just shoot them a quick email if we need to. I guess after thinking about it long and hard, I preferred the basic RSVP and sending a follow-up email vs. a questionnaire RSVP that people wouldn't even take the time to fill out.
haha, some of these RSVPs I've seen are more complicated than a tax return, that's all! =)
I copied Mrs. Onion's RSVP card (indicating meal selection by writing their initials)...

We just asked our guests to indicate the meal preference for each guest and then because my brother's girlfriend did the invites and such for us, we customized the number of spaces for names and options to the number on the invite so that it didn't look too much like a list.
Our caterer explained that this way things flow more smoothly without the waitstaff having to ask each guest what they said they wanted on the RSVP months ago :)
I opted for an RSVP card like Mrs Lemon's - I felt it was concise, yet informative (thanks Mrs. Lemon!).

On our RSVP cards, we have:
Mr./Ms.___________ (and then boxes to check for meal choice)
Mr./Ms.___________ (etc.)
At the reception, my placecards will be color differentiated by a specific stamp that goes with my theme. Each different color will refer to a certain meal choice.

All rsvp cards I have ever received with wedding invitations have you select the entrees but never ask you to specify who in your household is choosing what. I guess at most weddings, I've been seating with my parents or my date, so it doesn't really matter.
The coordinator at our hotel wants me to either have people list who wants what on the rsvp cards or to later call each guest to find out what they want. The latter idea seems ridiculous to me, so I far prefer the former, but I'm worried people won't get it (don't rsvp cards inexplicably cause enough confusion as it is?)
