Is the child invited?  I am working on the assumption he/she is, so go with this:

Outer envelope:

Ms. Jane Doe

Dr. Jack Smith

(Because they are not married, their names go on separate lines – the person you know best goes on top.  If you know both equally well, the female goes on top.  If they are married but have different names, they go on the same line, but he goes first.)

Inner envelope:

Ms. Jane Doe, Dr. Jack Smith, and Child OR Jane, Jack, and Child

You don’t necessarily have to put his name on the envelope.  You can address it to her and then include his name and the child on the inner envelope.
