Its really totally up to you, but I would make sure the RSVP is very easy to find. You don't need anymore excuses for people not rsvp'ing on time! ha!
I also did not use pocket fold or an inner envelope. In fact I didn't even do the ribbon. I did it in size order with the invitation being the biggest and it being at the "bottom" of the stack. Followed by the reception card, then the map, then the rsvp card (outside of its own envelope) on top.
Hope that helps!
I tied my invite, rsvp card and map with a satin ribbon as well. The lady at the store I purchased the ribbon from told me that traditionally, the rsvp and other inserts would sit on top of the invite, but I didn't like the idea of it covering the invite and our names so I just ended up placing the other inserts in back of the invite.

THat's how mine was The Evelyn on that page. It faced inward towards my invite, and was blank on the back.
Good luck!

I am not using pocketfolds or an inner envelope. My plan is to tie everything together with a nice satin ribbon.
So does the RSVP and map sit on top of my pretty invite?
