I tucked mine in the back of the invitation under the ribbon. Not too clever, but it worked and looked good. I put the return envelope on the top of the pile so that they saw that first.
I used both a 4" bellyband and a vellum inner envelope/pouch. Kinda overkill as either would have worked by themselves but it was pretty.
We actually attached the directions/parking info on the back of the invite. We used postcard RSVPs so there was no envelope. Plus, we had an additional accommodations card for only the oot guests. So those were the only 2 separate pieces inside the invite.
