I use Google Docs quite a bit and I always have a lot of luck creating a form to sent out in situations like this. The people you e-mail can typically fill out the form directly in their e-mail without going to a separate site and all the information will be collected in a Excel document.
IMO, one nice thing about this approach is that everyone won't see everyone else's information. You are basically posting your guest list online if you give people access to the actual document - and what if someone accidentally deletes information etc.
I don't know if you could send it out with the eSTD though - but, if you have to do two separate e-mails, you can just copy-paste.
I love your STD image. I don't really know what to do to the text portion though.
@eva - you mean you can embed the form directly into the email? When I created one, it sent me a link and that link brings you to a page with a form.

Sorry it took a while for a response... I have a big exam this week and I have been studying.
Here are some screen shots of the form and the results in my e-mail. Definitely make sure to check the box that says "Include form in the e-mail." In addition, I would recommend check the box on each question that says "Make this a required question" that way guests don't forget to give you some information. You can always try making a practice one and send it to yourself.

Help! We have our eSTDs ready to email, and I created a beautiful google doc for collecting precious addresses so we can send out real invitations someday (in the distant, foggy future... or in 4 months).
I'd really like to include a link to the address form with the STD, because that makes sense to me. Our STDs are an image which embeds in the email and links to our website (I love google labs...).
Trouble is that I am one of the most wordy people I know, and what I have now in the 'draft' of the email takes up more space than the STD almost! Ack!
"It would also be incredibly helpful to us if you could [click here] and enter your mailing address, or respond to this email with it, so we can send your formal invitation when the time comes. Thanks and we look forward to seeing you!"
Help! I cannot send that out in an email!
