Since your maiden name will be your middle name, on stationary you'd just write your last name: "John & Jane Smith".
If your maiden name and married name were hyphened, it would be "John Smith & Jane Doe-Smith"
One way of doing it is "Mr. and Mrs. John Smith", since you are opting not for the hyphen, but for your new legal last name as "Smith".
Really, you can choose to call yourself whatever you would like..

Like a lot of Bees seem to be doing, I want to keep my maiden name as a middle name and append my married name. So, FIRSTNAME MAIDENLAST MARRIEDLAST and use all three names as my official name.
So, how do you write those together on address/cards? here's an example: John Smith and Jane Doe marry...Jane takes his name but continues to use her maiden name.
Separately, they would be:
John Smith
Jane Doe Smith
On stationery, what would their names be?
John & Jane Smith?
John & Jane Doe Smith?
John Smith & Jane Doe Smith?
