Take a look at the Crane & Co. website, etiquette section, married couples, which covers this. They actually spell out "Doctor" which seems odd to me as you are not going to spell out other titles (e.g., Mr. and Mrs.) but who am I to say.

I'm about to address all of my envelopes but needed help with the wording and titles.
Do most of you prefer "Doctor" or "Dr." for example:
Doctor John Smith and Mrs. Mary Smith
Dr. John Smith and Mrs. Mary Smith
Also, if the wife is the doctor, how did you all go about addressing the envelopes?
Mr. John Smith and Dr. Mary Smith
Mr. John Smith and Doctor Mary Smith
Mr. and Mrs. John Smith
Please help!!! What have you seen?
