I replaced my middle name with my maiden name altogether, but I believe you can have as many middle names as you want. I took my marriage license in to the social security office, who just asked what I wanted my name to be, wrote it down exactly, and sent me on my way. Easy peasy.
I know lots of people who have done it! And now that its said and done, I wish I had done it myself. Like @gatorhailey said, you just write down what you want it to be and they change it!

So how does that work? Can I just add my maiden name to my middle name when I change my name? Cause I kind of like it.... but DH would be very sad if I didn't take his name. Anyone done that?
