I’m composing our menu cards (to be found at each place setting before the seated dinner and after our cocktail hour)
1. Do I include the passed appetizers that were served during the cocktail hour?
2. Where do I put “assorted rolls and butter” (currently I have it under the first course (with the salad)

I wouldn’t put the cocktail hour apps on there if people aren’t going to see the menus before the cocktail hour. The real purpose of the menu is to let people know what they’re about to consume (so they can be informed or avoid x or y). It might just confuse someone who’s then expecting another round of apps after sitting down.
I think a bread basket pretty obviously contains “rolls with butter,” so unless you want to specify the kind of bread you’re serving for some reason — like “sunflower seed rolls with asiago cheese” if you’re worried about people who hate seeds or have allergies or something — I’d vote for leaving it off the menu entirely. The caterer puts it on your menu just so you realize bread is included (On a side note, I saw one caterer charging 50 cents per piece for bread instead of having it included in a set price per head. Ridiculous!).
