Well, carat is actually a measurement of weight, so nobody will be able to tell you what the carat weight is on a set stone unless it was measured prior to being set.  With that said, your best bet would be to measure the (sapphire I presume?) stone in mm, and then use a comparison table such as bluenile’s or moissaniteco’s (which are based off of different diamond carat weights to approx. sizes) Best luck.
