How did this turn out for you? I want to keep my maiden name as a second last name, too. I'd love to hear if keeping 2 middle names is easy in Pennsylvania. Thanks for the info!
I had to go to the SS Office first with my current ID and a copy of our marriage certificate, then to the DMV.
And you were able to keep your original middle name and add your maiden name as a second middle name?
I'm just curious because some of the boards mention that some states require a court order to do this. I hope PA makes it simple! Thanks so much!
I'm really interested to hear more about this, too! FH has two middle names already, so I'm hoping that PA is pretty relaxed about 3 vs. 4 official names!
Hi Girls! I am now officially Jeni Rae Smith Jones AKA Jeni R.S. Jones (not my real name ;) ) ! It was pretty easy. It's been a while, but I'm pretty sure I went to the DMV first and then to the SS office. The guy at the DMV said something like "So we're changing it to Jeni Rae Jones?" And I said "Actually I want to keep all 4 names, with 2 middles." He didn't seem to mind at all.
I run into problems occasionally with places like the bank (who have my name as Jeni RaeSmith Jones) and at work (UPMC) who just have me as Jeni Rae Jones. But either way, all 4 names are on my legal documents. Hope that helps! Good luck!!

I wish I had known that when I changed mine... I would have kept my middle name too. =)
@mudbug: The least painful part of the name change is with S.S. and the DMV... The problem comes afterwards when you have to change it for ALL your other accounts, registrations, loans, etc. ;) I'm still working on mine and I changed it a month ago.

Hi, bees. I'm attempting to change my name today...
I want to keep all of my names, eg: First Middle Maiden Last, with two middle names.
Does anyone know in Pennsylvania if I have to go to the Driver's License Center or SS office first??
