It depends on what has worked for a patient before, also a lot of times women come in with a brand they would like to try. All of them will regulate your periods and make acne better. i try to start with a middle dose estrogen pill and adjust based on break-through bleeding and side effects such as nausea. The generic pills work just as well as the brand names. i try to stay away from tri-cyclic pills because I see no benefit in fluctuating doses and you cannot take them continuously. Basically I look for a generic pill, middle dose estrogen and go from there

I start with the low dose, monophasic and cheapest and work from there. If I know my patient has insurance, I do ones other patients find less nauseating
