I would get it now with your maiden name. You can change it to your married name after the wedding & honeymoon. If your passport has been issued within a year it’s free to change your name.

You need to run to a drug store and get your picture taken for the photo. If you do it at the passport office, they will charge you $30. If you go to CVS or a drug store, it’s only like $5. Then you mail it in with the paperwork. I read that it can take up to 3 months to get it back in the mail, but my fiance got his back in 2 weeks.

They will not let you have the passport under your married name until you are actually married and have the papers to show for it. They take the name from your birth certificate, and then, after you’re married, amend to whatever you ask upon seeing your marriage certificate.

Ditto to everything that’s been said here–plus, if there is a government shutdown this month because the budget is not passed, it will slow down passport processing.

If you’re worried about time you can pay an extra fee to get your passport rushed. As of now, normal passport processing takes 4-6 weeks and rush processing takes 2-3 weeks. If you are really in a bind, you can make an appointment to go to a passport processing center in person in the two weeks before your trip, and they can often work miracles to get your passport to you in time!
