I used Shutterfly and it worked out very well. The only thing I suggest is to leave LOTS of white space for guests to write on. I specifically wanted them to sign near a photo. Guests were hesitant. I recommend leaving blank white pages as Mrs. Emerald did. Also, make a "Please sign our guestbook" sign. No one at our wedding knew to sign it.
I did one using Picaboo, but I wasn't really thrilled with the print quality. (Photos and text were kinda grainy looking, and the first time around, there were some really bad quality issues with the "softcover" option -- creasing on the spine before it had been opened, etc.)
However, a plus for them is that they are the only place I found (maybe someone else knows more) that does small size (typical guest book size) HARDCOVER (fabric covered) books. Everyone else's small, rectangular books were only in softcover (paper).
I used our engagement photos on the left hand page and lined pages on the right. I mostly did it b/c I didn't like any of the commercially available books for various reasons, including my own weirdness about how the lines were spaced/printed.
I think I would've done it differently (see my post in the "creative cards" thread) in hindsight. We only had 90 guests, and so only 2-3 pages were signed. I wanted people to put name/address per usual, and also write us a message. Hubs said "yeah, right, no one's gonna do that." ...Sigh. He was right. (Of course, I didn't tell our guest book monitor to tell them that, nor did I have a sign, so I've only myself to blame.)
i'm doing mine through iphoto... just putting a few pictures on each page so there's still a lot of blank space for people to write their comments in. Even better? They print your book right away and ship it within a few days of receiving your order, so for last minute procrastinators like me, there's plenty of time to get around to doing it!

There has been mention of photo guest books in some comments/blogs. How are you girls putting these together? I've done photo books on sites like iphoto before...are you just making a book with some blank text pages added in for guests to sign on? Anyone have anything specific they've used that they really liked? My fiance wants to do a plain guest book but I'd like to spruce it up a bit!
Any suggestions or anecdotes would be appreciated!
