I think most often the names are written as they are pronounced-e.g. John Smith, although they may be displayed alphabetically by last name.

We just did first names (and last initials if necessary). If you do Mr. and Mrs. together, it’s more formal, but Smith, John just seems stiff. Match your style to the formality of the event and the escort cards.
