i would get rid of the “__ of __ attending” line and just fill in the “M_______” for each person’s entree choice.

alternatively, keep the “__ of __ attending” and get rid of all the “M___”s..instead:

please inital each guest’s entree choice:

____Chicken _____Beef ____Children’s

Is your wedding more formal or casual?

Formally, I would change the date to “the fourth of August”

But I like formal and traditional so that’s just me 🙂

Do you really need to know what EACH person is having?  For our cater we just needed to know how many of each meal were going to each table.  And if people are on the same response card they are likely sitting at the same table.  So if we knew a couple wanted one chicken and one beef it really didn’t matter which one wanted which because they just let the waiter know what they ordered when they came around.

I second kitzy’s response. Have at the bottom for them to initial beside their entree choice. We are just starting to get our RSVPs back and it’s been pretty successful. 

We are having a plated meal and the caterers need have a breakdown, per table before the event.

If you want to avoid people bringing additional guests, if possible I would include all the invited guest names on the card.
