Here is mine - it's working out really well so far. No one has been confused and I have gotten all the information I needed from everyone.

For my own response cards, I didn't even think about this being an issue (but now it kind of is, because I don't know who ordered what in each family). But I just got a wedding invitation from someone else and here's what she did:

This is how i set up mine. I also had different versions for ppl with kids and singles. I added the cell phone part because our location is outside and our plan b is in a different location.

We are giving our guests a choice between four meals on the invitation response cards. I used some suggestions I found online to see how the response cards should look and they came out a bit like this:

I wanted to make place cards for the reception for each individual guest showing their meal choice. So this is the question: If a family of 4 responds with "2 beef" and "2 chicken" how do know who ordered which meal???
