Ask his mom what she thinks would be best. ![]()
In my experience, they sell safety pins that are super duper kid friendly. But- if his mom knows him best, I might be willing to bet he might grab the flower and tear it to shreds and try to throw it like a baseball. I know far too many three yr old boys that would do this.
Maybe make him a special little fabric flower boutineer...
I wouldn't use a pin with a three yr old though. I share your concern with that.

My ring bearer is 3 years old (no flower girl because we have no little girls in the family). Do I need a boutineer for him? I am only worried he will stab himself (or more likely others) with the pin. Should I use a safety pin?
