I think I like the first one better, it's less work for the guest, BUT.. if someone has the same initials that would be a problem.
I like number two. Straight to the point and no room for confusion :)
I think the first one is definitely a more succinct, and therefore better set up. But, I think you may have to be more direct in your "directions" Most people will not bother reading, and will just place check marks, which could cause a huge headache for you. (In a hurry, that's what I would do). So perhaps "initial" in bold or set it up differently.

Our venue requires us to know what each person will be eating. (In other words, I can't say Jack and Jill want 1 chicken and 1 beef, I have to say Jack wants beef and Jill wants chicken.) We're trying to find the best way to word this on the RSVP with as few mistakes as possible.
So, if you were a guest, which one would be easier to figure out? Or another suggestion? Any and all suggestions are VERY welcome! Thanks!
