Weddingbee boards
Do a blank card. We did (because that is how the set came). It was so much fun getting everyone's response. Though I think some of the guests thought it was a bit of a nuisance not being able to just check off a box.

Our yes or no's are going to say:

It lessens the blow when someone declines :(
Mine say:

___ Decline with regret
___ Number of people attending
We've had so many people write little replies and notes and smiley faces! It's pretty cool.
we are having a beach wedding so our responses are
___in port (accepts) ___lost at sea (declines)
we are also leaving room for guests to make some music requests by saying
"make a request and tell us which songs make you get up and dance"
I have heard of people asking their guests for advice on love and marriage and then displaying the responses at the wedding.
For RSVPs, I decided to take a chance on doing Mad Lib ones. I have quite a few favorites! Here's one of them:

Ours are pretty standard... I am having an etsy vendor make them for us and they're totally perfect (in style AND price)! :)
We added the "We have reserved ___ seats in your honor" line to help manage our guest list! I don't need, or want, any surprises! 
@jocelyn3476, that's a really cute idea! I would do that too if I wasn't such a control freak lol!
ours are standard as well. We asked them to respond by October 3, then put a place to put the names in, then check boxes for accept/decline, then check boxes for the food choice. Nothing creative here!
