They go out before you send your invites. If you have out of town guests coming sending them as soon as possible is a good idea. We don't have out of town guests and just sent ours out (5 months before wedding).
It really depends. If it is over a holiday weekend or in a crazy location or you have a lot of out-of-towners, you could send 9 months or so before. Otherwise six months to four months before is just fine. You probably wouldn't want to send it out with your invites because the point of a STD is to let people know about the event long before you are sending out invitations.
90% of our guests were OOT and our wedding is on a Friday, also many of our friends are physicians and they needed to know the dates well in advance for scheduling purposes...so we mailed our STD 7/8 months before. Typically its 6 months...and I've even received some 3 months before the wedding, which I thought was pointless because we got the invites a few weeks later!
I concur with aout 6 months, a little sooner if you have a lot of out of town guests or it's a busy weekend, possibly a little later if everyone is mostly in town and it's a generally quiet weekend. But definitely between 4 - 9 months out.

How soon is too soon to send out "save the dates?" or do those just go out when you send the invitations?
