I bought my crystals off of ebay. There is a huge selection to choose from at really reasonable prices

I bought mine off ebay, too, but rhinestoneguy.com is also a good place to buy.  He has information on this page about how many rhinestones you would need per sq. mm:  http://www.rhinestoneguy.com/HowToRhinestoneCell.html

I ordered all of mine from dream time creations. com and  I found their bulk price to be pretty good and they are real swarovski.

I bought all of mine from Dreamtime Creations.

As far as how many crystals you need, it depends on quite a bit. How many different size you use (sticking to the bigger ones means less crystals but also less sparkle), what style of shoe and what size you are.

For example, I have done two pairs of pumps for myself (no platforms, 3.5″ heels). I wear a size 9 and it took me about 9,000 crystals per pair using size 5ss, 7ss, 9ss, 12ss, 16ss and 20ss. I did a pair of 5″ heel, platform, slingbacks for my sister that are a size 6.5. I used the same array of sizes and they also took me about 9,000 but probably would have taken about 5,00 or 6,000 with a lower heel and no platform.
