Did the gift come with a card?  If so, send the thank you to whoever signed the card – whether both of them or just her.

Well that is not helpful!  If you did not know her date prior to your wedding, I would guess that the gift is probably just from her.  In which case, I might just send the thank you to her (you probably don’t have her date’s info anyways).  BUT in the message part of the thank you, you could put something like “Thank you and <date> for coming to the wedding!  It was wonderful to see you again and I am so happy you were able to make it.” in addition to the “thanks for the <insert gift here>” part.  Then it also includes her date and you should get away with it regardless of who the gift is from.  I would be ok with that if I were the “date” in this situation and I had contributed a bit.

I would address the card to just your friend, since that’s the person you have the relationship with and there was no attached card.

I would do just your friend, but either way is fine.
