The guest writes their names on the line.  So if Mr. John Smith and his girlfriend, Jenny Blue are coming they would write:

Mr. John Smith and Miss Jenny Blue

I debated about including the M on the cards, but after some research decided to go with it. If there’s just a line, people may write only the first name and you may be confused about who sent the RSVP card. Also if you have MD’s or PhD’s attending who use Dr. as part of their social salutation, they are just supposed to cross out the M and write in “Dr.” Hope that helps!

I had heard a lot of complaints that people didn’t know what to do with that M_____ when they were filing out the RSVPs so they left it blank. So when I made my RSVP cards I did Name(s)______________ so everybody knew just what to do.  We didn’t get a single one back without a name on it.  It was also helpful to do this because about a quarter of our guest list are Dr. rather than Mr.
