Make sure you take your old SS card! I know when I went, they tore mine up on the spot right in front of me!
You should bring your marriage license, social security card, and a photo ID. Just go to SS first to change your name before the DMV.
I think you'll need your marriage license, driver's license plus one other form of I.D. You'll be filling out form SS-5 - which has instructions. Here's a link:

Unfornately, I don't have my original SS card....my mother has been unable to locate it. =(
Do I need to bring this in? Is it a must?
I just changed mine last month and I only needed my driver's license and marriage certificate (an official notorized copy). I actually had filled out the form mentioned above, and the guy at the office told me, "I don't need that" because he typed it all into the computer. I recently misplaced my old SS card, but he never asked for it, so it wasn't a problem!
It depends a lot on the office you go into. Our local office didn't even ask to see my old ss card, but some places do. Your best bet would be to call your local ss office and ask what they recommend you bring in. It really seems like different places ask for different information/identifications.

I was just in the SS office in downtown D.C. last Friday because (embarassingly enough), I never got around to legally changing my name back to my maiden name after my divorce. It's the same form (SS-5), and they definitely had me fill it out. I brought my passport along with my divorce decree, and that was enough for them; they didn't require my old SS card either.

I guess it depends on where you live, I mailed in my form to the closest SS office to me, but I did call before hand and asked what I needed to include and all I needed to send them was the SS-5 form and my marriage license. I would give them a call and see what you need before going down there! I receieved my SS card in the mail just last Friday just a week after they sent in the information! Good Luck! I think I'm done with everything finally!! Guess a month after starting the name change isn't really too bad!!
Along the same lines, what do I need to bring to the DMV? Do I have to have received the new SS card? (Hoping not, since I have to get new license plates before the end of the month and want to make only one trip.)
When I went to the DMV in Alabama, they only needed my old license (from CO), social security card (with Maiden name) and my marriage license in order to change my name and get a new license. I would check with your local DMV to be sure though.
I took my DL, my old SS card, our marriage license, and my birth certificate. All the lady looked at was my DL and SS card.
I brought my old SS card, DL, passport, and marriage license, but they only looked at passport and my marriage license.
I actually went to the DMV before I sent off for my new SS card. All they needed from me was the marriage license and I was good to go!
You might also consider bringing a sandwich and a loooong novel. Also sunglasses and headphones so you don't have to talk to anybody there. Just sayin'....

This may sound like a dumb question..(sorry!) but I want to make sure I have this right before I change my information for my SS card.
What all information do I need to bring with me to change my name? Do I just need to bring my marriage license and Driver's license? Is there anything else I'm forgetting?
