I'm going to answer my own question!
My centerpieces will be a simple iris bouquet surrounded by silver pillar candles. Since they include flowers, my florist will be "putting them together" for me, but I still have to find the candles and holders, so I sort of consider it DIY.
I'm doing all my own stationary (invitations, escort cards, wedding programs, thank you notes, etc). I ordered some pre-made pocket fold cards from paperandmore.com and some rubber stamping supplies from ebay, and I also raided my mom's massive scrapbooking stash for ink and other materials. I plan to print the invitation wording on my printer, then use double-sided tape to attach that to the pocket folds, and embellish with rubber stamp art.
I also plan to order some card stock and have my wedding programs printed up, then decorate them with ribbon and a mini carnation after this inspiration:
I'm DIY'ing a lot! But I'm still pretty far out, and also I'm kind of a fail-bride in terms of planning, so I don't have too many specifics yet.
Flowers: I'm getting flowers wholesale and doing the bouquets. I'm not sure yet if I"m doing the bouts. I might outsource that since they'll be a lot cheaper.
Centerpieces/all decor: Not sure what I'm doing yet... but whatever it is, I'm doing it myself! I think there will be paper lanterns hanging from the ceiling and the centerpieces will be some combo of vases, candles, maybe branches.... yea I dunno. :)
Favors: Making my own chocolate covered pretzel rods. Mmmm
OOT bags: Not sure whats going to be in them... but I'll be doing them myself.
Wedding party gifts: No clue what the groomsman are getting, but I'm making quilts for my MOH and BMs
Paper goods: STDs, Invites, Programs, Escort cards, etc. All me.
DJ: FI is taking care of the ipod stuff.
Looking at this list.... I really need to get moving....
For the most part, it's just the paper products (invitations, STDs, programs, escort cards, rehearsal dinner invitations, photosharing, table #'s, etc). I'm also making bridesmaid gifts and pew bows for the church.

LOL @CorgiTales It looks like you have your work cut out for you! Chocolate pretzel rods sounds great! And quilts for your BM's? That's a lot of work, but it will be a great personal gift since you made it yourself. A word of caution about bouquets: They typically arrive several days before-hand, and you need to keep them refrigerated until the day before when you put them together. I highly suggest enlisting the help of friends for this, because it's last-minute kind of work. Good luck!!

I really didn't even know where to start with DIYing except for all of my stationary/paper products! You ladies really have a lot of great ideas in mind. So I guess after realizing how much was possible, I will also be DIYing my candy buffet and containers, my escort cards (we're considering doing a seating chart instead of escort cards), my STDs, invitations, TYs, and possibly my bouquets.
We DIYed our invitations, favor donation cards, table numbers, direction cards, and pew bows.
I want to DIY my invites&co but not sure whether I'll be able to juggle it while finishing up this semester at university (and the invites just need to go out before the end of the month).. Apart from that I am going to make my own favors, guest book and our wedding album.

Here is my list:
I am DIYing every single thing. I even have a friend that is a pastor that is performing our ceremony, and a friend that is a photographer that is taking photographs. I have to get my alterations done, so that's the one thing I am not very good at myself. I even did our engagement shots, processed them, printed them, toned them...ugh. It was a lot of work, but they'll be on display at the wedding, and I'll give them as gifts, so they were worth the time and effort and money.
I am glad that there are other DIYers out there! Keep up the good work ladies!
So far we've DIY'd the invites, the favour boxes, the photo sharing cards and the wish cards for the guestbook. I've also made 930 cranes for decor and plan on finishing them soon. We also plan on making fudge and brownies for the favours (closer to the time) and I need to finish making the board to put the wish cards for the guestbook up. And we'll be semi-DIYing the thank you cards after the wedding as well.

@noritake22: do you have any tutorials to share as far pomanders are concerned? I'm having a hard time finding cute ones! Also, what are you going to include in your bathroom baskets?
Gees, I didn't think I was DIYing much until I checked like half the boxes on your poll!
My BM is designing all of our paper products (invites, programs, escort cards, etc) but we're printing and assembling it all ourselves. I guess that makes a lot.

I really have to give some of you ladies props! Holy cow! I don't think I could even begin to tackle that many projects.
I'm DIYing favors, OOT bags, STDs, our full invite suite, my headpiece and our chapel & reception site decor. I may also add the centerpieces & bathroom baskets to my list depending on how things look time wise. Not as ambitious as some bees but a lot for me!
I dont have pictures yet, but I am DIY-ing my invites and reception menus and I am cooking the food myself, we are only having 20 people so its easy to do. I am making the floral arrangements and centerpieces. I am making my hairpiece, I will post pictures as soon as I can
We have done a good number of DIY stuff I think, but we are not nearly as talented as some of the folks I have seen on here!
No pictures of the invites, but they were simple flat white cards with black raised lettering. FI and I poked holes and added black and white polka dot (theme of the wedding) ribbon to the outside edge of the invites.
Our favor boxes were made by my FMIL...the tops fold into hearts...adorable!

Our centerpieces will be DIY...bought 45 fluted bowl vases from Walmart, filled them with clear/iridescent and red accent gems, and tied black and white polka dot ribbon around the bottom. They will be filled with 6 white roses, cut short so the flowers sit just at the fluted rim of the vase. This will be on a black overlay, on a white tablecloth.
The final DIY was our "guest book" which is more of a well-wishes and advice station. I took a larger bowl vase (pretty much a fishbowl), filled it with the same clear and red accent gems, tied the black and white polka dot ribbon around the top and bottom of the bowl. I used a FREE Avery template for monogrammed business cards that I personalized to our monogram. The business cards are blank other than our monogram, and the guests will write us well-wishes and advice on them, to be placed in the larger bowl. After printing almost 300 cards (1 package is 100 cards, around $3), our guest book is ready to go!
I checked the stuff that applies to us, but we're also serving homemade wine at the reception.
I had most things professionally done, but I made my card box and decorated the church. I would have loved to do more things, but I'm glad I didn't because of the added stress.

Umm, all of it. We did everything from setting up 127 paper lanterns with various forms of lighting to bringing in the chair covers, chair sashes, and tablecloths. We also made our own chuppah (wedding canopy).
Catering (i.e. your family is cooking the reception food) :
We hired two friends to do most of it. However, my wife made the wedding cake, and I came up with the signature drink.
DJ services (i.e. iPod) :
Although we had a musician at the reception, the ceremony music was a CD we compiled ourselves.
