I know some of you are like, "who's the downer?", but there's a valid reason for the question.
I work in a program that works soley with victims of the most violent and heinous crimes... super uplifting stuff. Every year there's a week in April called National Crime Victim's Rights Week. This is huge to us. So we do a community wide vigil/commemoration/presentation kinda thing. We have choirs and survivors and performances. I am always in charge of all things visual. So each year, I have to make a slide show of pictures that family members send me of their loved ones who have been murdered. It's actually a horrible experience each year because I cry and cry and cry but, it means the world to those families.
I always put songs into the slide show. Obviously it can't be like "Oh Happy Day" so I am looking to the hive for some suggestions. So it doesn't necessarily have to be the saddest song you know, but that tends to be how it goes.
In years past I've had, "Its so hard to say goodbye to yesterday" by Boyz II Men, "Please remember" by Leann Rimes, and "I will remember you" I think by Amy Grant. The unfortunate thing, is that there are so many victims, I need more than one song this year. Oh man, I wanna cry now.

For Good, from the musical Wicked. It gets to me everytime I hear it!
Ditto I Will Remember You when it's on the ASPCA commercials... gets me every time.
For me personally, Baylon by David Gray, Somewhere Over the Rainbow, and that religious song I Can Only Imagine (funny, because I'm not religious, but it played at my grandma's funeral) all have personal meaning related to people I've lost.
Ave Maria always gets me. I heard it at my FI cousin's funeral and just bawled.

Garth Brooks - The Dance. I know it's "country" and old but it makes me bawl like a baby every time I hear it.

I love that song I'm Already There By Lonestar. It's sad but at the same time I think it's kinda uplifting.
Ugh, Tears in Heaven gets me every time. I seriously refuse to listen to that song ever. It's an instant downer. It's too bad because it's also a really beautiful song.
It doesn't work for your theme, but has anyone heard that "Christmas Shoes" song? I heard it for the first time on the radio before Christmas on my way to work. I was a sobbing mess by the time I pulled into the parking lot.
Not sure if the lyrics are right but "If you're gone" by Matchbox Twenty is sad and wistful to me.
Oh, Tears in Heaven is a sad one. Cats in the Cradle is the one that gets me every time. Also She's Leaving Home by the Beatles. There's one or two others I can't recall right now. Family relationships and the passage of time are the only themes that actually hit me emotionally.
For me personally b/c it reminds me of my mom and while she is still physically here - she is terminally ill w/ a neurological disease so she's pretty checked out...

Second vote for Hallejulah. The saddest song to me personally is The Promise by Tracy Chapman, and I think the lyrics work for your cause/project.
Oh My God, Jars of Clay. A six minute song that makes me cry every time I hear it. I don't think it will work for what you're doing, but you may find a use for it. It doesn't seem like much at first; just listen to the lyrics and hold out to the end.
Oh Tears in heaven is a good one! And jackie-o's suggestions are great too.
Holes in the floor of heaven, Steve Wariner. It would definitely fit your theme.
Songbird by Eva Cassidy, Blackbird by the Beatles (there's also a really pretty version by Sarah McLachlan), Angel Standing By by Jewel, You Are Not Alone by Patty Griffin.

Oh... and On Saturday Afternoons in 1963 by Ricky Lee Jones. I heard that on an episode of 'House' once and bawled.
The only song I think that ever makes me cry and might be ok for your theme is Gravity by Sara Bareilles. It's beautiful.
"Angel" by Sarah McLachlan would be fitting.
I have never heard "The Rose" by Bette Midler without crying- every. single. time.
I love Jeff Buckley, so I like Halelujiah too.
Also, "Fix You" by Coldplay makes me cry. The lyrics kill me.
"Amazing Grace" also an obvious good one.

......And I just looked up "The Rose" online and now my eyes are welled up with tears at work. LOL.
i know! tears in heaven was sad to begin with, but then when i learned that it is a tribute to eric clapton's son that died it was even MORE sad
Evanescence: My Immortal
It was played at a friend of mines funnel who died our senior year of HS. It has such a creepy feel to it and totally brings me to tears hearing it. His name was Evan so even the bands name reminds me of the entire thing...Evan's escence. Can't even listen to them
We also played Run by Snow Patrol for a friend who recently passed- I don't know if its exactly what youre looking for but it worked for us.
